As patient, how do you change your name, date of birth, address Print

If you have entered the wrong name or date of birth at registration, or any other of your details or address have changed, you can change these details by following the steps below.

Log into your account by going to https://engage.gp/#/login or from your practices website and clicking to consult online.

Once logged on select 'My account'. And then Select 'Personal Details'.

Make the desired changes by selecting what you want to edit and click save.

The changes will be saved and your display name and other details will be changed.
